Please arrive at your scheduled appointment time approximately 15 minutes early. You may download and fill out the “New Patient Forms” online and bring them with you to your appointment. This will save you time during the check-in process. Please remember to bring your insurance provider’s card and a current identification card to your appointment. We will use this to bill your insurance for your visit. If you are seeing one of our physicians for a consultation, second opinion or simply as a new physician, please bring your medical records with you and any other pertinent reports (i.e. ultrasounds, CT scans, labs, etc.). It is also a good idea to bring a written list of any medications you may be taking or have recently taken. If you are unable to make your scheduled appointment, please be courteous and call us to cancel that appointment at least 24 hours in advance so that we may use that time for someone else. We will do our very best to notify you in advance of your appointment should our daily schedule become interrupted due to unforeseeable hospital or surgical events. Also, please be aware that it may take up to 2 weeks for you to receive any results from your visit. This includes blood tests, mammograms, ultrasounds, pap smears and other reports. We attempt to process the more urgent results first. We do, however, attempt to notify all patients of all results, even if they are normal.
